IRWIN TOWNSHIP, Pa. (EYT) — Franklin-based State Police have released the name of a Polk man who died as a result of a three-vehicle crash that occurred around 5:35 a.m. on May 21, on State Route 8, on Richard Frame Memorial Highway, 1985 feet south of Georgetown Road, in Irwin Township, Venango County.

Police say 35-year-old Holly M. Eppinger, of Fredonia, 61-year-old Robert C. Shaffer, of Polk, and 22-year-old George C. Hannah, of Hermitage, were involved in the collision.

According to police, Eppinger was traveling north on SR 8 when she crossed the center line and off the western edge of the roadway. Her 2001 Dodge Grand Caravan then traveled back onto the southbound lane of SR 8.

The vehicle’s passenger side front bumper area had an initial impact with Shaffer’s 2000 Ford Ranger’s passenger side front bumper. After impact, the Dodge Caravan rotated counterclockwise and impacted a 2004 Buick Le Sabre, operated by Hannah, which was traveling south on SR 8, police say.

The Ford Ranger traveled in an easterly direction and came to a final rest east of SR 8. The operator, Robert C. Shaffer, was pronounced deceased at the Grove City ER by the Mercer County Coroner, according to police.

Eppinger was using a seat belt and suffered moderate injuries.

Hannah was also using a seat belt and suffered minor injuries.

Superior Ambulance assisted at the scene.

Eppinger was charged with a traffic violation.

The investigation remains open.
