Forgery, Tampering Charges Filed Against Oil City Mom Who Gave School Fraudulent Medical Excuses

OIL CITY, Pa. (EYT) – An Oil City woman is facing forgery and tampering charges after she allegedly forged medical excuses for her three children.

Court documents indicate that Oil City Police Patrolman Mitchell Labesky filed criminal charges against Jacquelyn Nicole Conway, 28, on Friday at Magisterial District Judge Andrew F. Fish’s office.

According to a criminal complaint filed at Judge Fish’s office, Oil City Police were dispatched to Seventh Street Elementary on Dec. 12 around 11:00 a.m. to investigate possible forged or fraudulent medical excuses being turned in by a parent.

The criminal complaint states that the school gave police three medical excuses provided by Conway. The school noted that the children have had truancy issues in the past. The excuses had each of Conway’s three children’s names on them and a signed doctor’s name in the lower right portion of the excuse.

Further investigation revealed that medical excuses were forged.

The criminal complaint states that police were able to obtain a sample excuse from the medical facility listed on the excuses and the document in “no way matched the excuses turned in by (Conway).”

Conway later admitted to police at she “had the medical excuses made up,” according to the criminal complaint.

“(Conway) stated she did not want to get into more issues with truancy and did not want to have to go back through the court system,” said Ptlm. Labesky in the criminal complaint.

The complaint continues, “She stated that she and a friend came up with the medical excuses and she turned them into the school on Dec. 11.”

Police subsequently filed three counts of first-degree misdemeanor forgery and one first-degree misdemeanor count of tampering with records against Conway.

She faces a preliminary hearing on Jan. 23 at 8:30 a.m. with Judge Fish presiding.

It’s unclear if charges will be filed against the person who assisted Conway.
